UC SHS, JS and GS Guidelines for Enrollment SY 2020-2021

  1. For Incoming Grade 11 and Transferees
    • Accomplish the Admission Form:

      a. Log on to UC SHS Admission using any web browser, OR

      b. Download the form and email accomplished form to [email protected]

      Please upload/attach the following along with your Admission Form:
      • Photo of Grade 10 Report Card or
      • Photo of Grade 10 School ID (if Report Card is unavailable)
    • Pay enrollment fee (Php 6000.00 if not reserved, Php 4000.00 if reserved) via over-the-counter bank deposit or electronic fund transfer to any of the following accounts:

    • Email proof of bank payment with the student’s name to [email protected]. A confirmation email shall be sent within two (2) business days along with the student’s Official Registration Form where the student’s ID Number shall be indicated.

      Note: Other admission requirements shall be submitted within the first trimester of the school year.

      • Form 138 (Grade 10 Card) with Learner Reference Number
      • Copy of Birth Certificate (PSA/NSO)
      • ESC/QVR Certificate (for Private JHS completers)


  2. For Incoming Grade 12 students
    • Accomplish the Admission Form

    • Send accomplished Admission Form to your Class Adviser’s Messenger Account.
    • Pay fees via over-the-counter bank deposit or electronic fund transfer to any of the following accounts:

  3. For Incoming Grade School & Junior High School students
    • Note: For new students, please upload/attach thru email the following together with your accomplished admission form

      • For Grade 1: Photo of Result of ECCD Assessment Checklist
      • For Grade 2-10: Photo of SF9 (Progress Report Card

    • Pay fees via over-the-counter bank deposit or electronic fund transfer to any of the following accounts:
